Membership Details
- The membership fee is $4,200 per year, paid annually or quarterly
- Dr. Teper is an Internist and sees only adult patients, ages 18 and up
- Except for the annual wellness visit, insurance is billed for each office visit and for additional services
- Office staff will collect your insurance co-pay at the time of visit
- The membership fee does not include care provided by other physicians, hospitalization, medications, or any other outside services
Care
- Practice limited to 350 patients
- 90 minute annual wellness visit, including lab work (at no extra charge); review of results and a comprehensive health plan
- Same or next day expanded office visits in an unhurried environment
- Direct 24/7 access to Dr. Teper via cell phone
- House visits, when medically appropriate
- Hospital visits
- Skilled nursing facility/acute rehabilitation visits
- Coordination of care with specialists and consultants
- Communication with family caregivers, when requested
